每一個意外事件的原因都可以分為兩層。顯性（active）錯誤：直接觸發意外的人事物。另一個更重要的層次是潛伏（latent）錯誤：事件脈絡中存在已久的設計、管理、組織與文化問題。人們容易注意到顯性錯誤，卻往往忽略更重要的事實：因為存在著一系列的潛伏錯誤，顯性錯誤才可能引發意外事件。

日本福島核災事件獨立調查委員會七月發表了正式的調查報告。委員會主席黑川清在報告的序文中把潛伏錯誤的層次解釋得很清楚。他說，這是一場「日本製的災難」。事件最根本的原因源自日本文化：對權威反射式的服從、不敢質疑權威、對規則的盲目遵守、集體主義、以及島國的孤立性格。這就是為什麼他在接受訪問時說：「雖然在任何地方都可能發生，但又以日本更容易發生。」
