如何辦理體格檢查及體能測驗？

(一)辦理程序：
1.「汽(機)車駕駛執照登記書」(體檢表)1份，填妥表格內個人資料，並黏貼本人最近6個月內拍攝之1吋光面素色背景脫帽五官清晰正面半身黑白或彩色照片2張，並不得使用合成照片。
2.體格檢查及體能測驗應由公立醫院或衛生機關或公路監理機關指定之醫院、診所、團體為之。
(二)合格標準：
1.體格檢查：
(1)視力：兩眼祼視力達0.6以上者，且每眼各達0.5以上者或矯正後兩眼視力達0.8以上，且每眼各達0.6以上者。
(2)辨色力：能識別紅、黃、綠色者。
(3)聽力：能辨別音響者。
(4)四肢：四肢健全無殘缺者。
(5)活動能力：全身及四肢關節活體靈敏者。
(6)疾病：無精神耗弱、目盲、癲癇或其他足以影響汽車駕駛之疾病。
(7)其他：無酒精、麻醉劑及興奮劑中毒者。
2.體能測驗：
(1)視野左右兩眼各達150度以上者。
(2)夜視無夜盲症者。
(三)有效期限：自體檢合格日起1年內，逾期無效。
(四)費用：新臺幣150元。
