《醃頭案》醫師：陳佳富疑有靈魂出竅現象

〔記者王善嬿／嘉市報導〕嘉義地方法院昨審理醃頭案，涉嫌殺害胞妹的嫌犯陳佳富面對法官訊問多行使緘默權，到場鑑定的精神科醫師周士雍認為，陳上次開庭口吐白沫，可能是「解離」症狀，陳的辯護律師要求再次精神鑑定，檢方則認為沒有必要，法官裁決交由合議庭評議決定。

昨開庭時間長達3個多小時，陳佳富有別以往，神情較為鎮定，一開始僅嘟嘴、低頭，直到勘驗影片才略顯緊張，雙手不停捲褲管，搖頭拉上衣，但精神狀況正常，院方為防萬一，還請嘉義長庚醫院精神科主治醫師周士雍到場鑑定。

周士雍指出，經開庭前兩次到看守所訪談，及參考陳佳富填答的量表和開庭表現結果發現，若非陳佳富說謊，就可能有像靈魂出竅情形，醫學上稱「精神解離」。

他說，解離就是人在現場，但意識卻不在，像睡著一樣，可是又沒那麼舒服，解離程度到極致會人格分裂，但因缺乏旁人觀察資料，目前只能假設，尚無法斷定。

服用百憂解等抗憂鬱劑會增加上消化道出血機率。台北榮總胃腸科醫師盧俊良研究團隊發現，精神疾病患者若服用「SSRI血清張力素回收抑制劑」這類抗憂鬱劑藥物兩周，相較於未用藥者，其上消化道出血機率會增加近一倍。

研究主要作者是北榮內科部醫師王彥博及腎臟科總醫師陳永泰，他們蒐集一九九八至二○○九年健保資料庫，共十八萬七千多名精神疾病住院患者，從中找出第一次腸胃出血有五千三百七十七人，進一步分析患者出血前一個月的用藥情形，分為服藥七、十四及廿八天共三組，相較未吃SSRI抗憂鬱劑的精神病住院病患，十四天內服藥會增加百分之八十四的食道、胃及十二指腸等上消化道出血風險，七天及廿八天內服藥則增加百分之六十七。

不過，王彥博說，SSRI劑量多寡並無太大影響，顯示只要吃抗憂鬱劑就會增加上消化道出血機率。此外，研究還發現，過去有胃潰瘍或胃酸逆流等上消化疾病、年紀小於五十五歲以及男性，服用SSRI抗憂鬱劑更容易發生上消化道出血，甚至較沒吃藥的精神病患多一至兩倍。這項研究成果近期將刊登在美國知名精神科雜誌（American Journal of Psychiatry）上。

王彥博表示，傳統抗憂鬱劑的副作用多，患者易出現心律不整或排尿困難、性功能障礙等症狀，新藥SSRI是目前常用的抗憂鬱劑，國內使用的SSRI藥物約六種，百憂解是其中一種。上消化道出血的主要症狀包括貧血、黑便、血便等，治療可服用陽離子幫浦阻斷劑或接受內視鏡止血手術。

台安醫院精神科主任許正典表示，SSRI與血清素有關，會改變腸道吸收及調節能力，容易促進胃酸分泌增加或減少，門診有人因此拉肚子或便秘。此外，人體對外來血清素的反應較大，補充SSRI可能增加胃破皮或出血情形，因此精神疾病患者用藥治療，一開始不建議服用太高劑量。

許正典說，精神疾病住院患者多屬病程較嚴重，除了使用抗憂鬱劑外，睡不好也會影響腸胃系統；如果又服用安眠藥，恐致腸胃蠕動變慢。
